Sesame / Success by Sesame Review: Following the Care From Video Visit to Pharmacy

Success by Sesame connects patients with independent clinicians for weight-management care. Its documented pathway includes video evaluation, messaging and prescription support, but medication purchases, pharmacy fulfillment and several billing terms require separate attention.

The most important handoff in Success by Sesame is not the first questionnaire. It is the transition from paying for access to a clinician to obtaining an appropriate prescription, finding the right pharmacy channel and arranging continuing care. Sesame describes a connected weight-management program, but those steps involve different responsibilities. Independent clinician practices deliver clinical services through its platform; Sesame itself is not a pharmacy. A subscription buys documented care services, not a guaranteed prescription or an included supply of GLP-1 medication.

This review follows that pathway using public documentation consulted October 19, 2026. The program offers video evaluation, provider messaging, weight tracking and help with medication authorization paperwork. It also publishes meaningful exceptions: laboratory coverage depends on location, advertised membership prices can vary by clinician, and medication costs sit outside the subscription. Conflicting descriptions of billing intervals, cancellation and insurance-related eligibility deserve resolution before enrollment. The practical question is whether the clinical, payment and pharmacy arrangements fit together clearly enough for a patient to know who handles each next step.

Start with who is actually providing care

Sesame, Inc., also doing business as Sesame or SesameCare, operates through its website and mobile applications. Its homepage says affiliated practices are independently owned and operated by licensed clinicians. That makes clinician selection a substantive part of enrollment, rather than merely choosing an appointment slot. The platform provides access, while the selected practice carries out the clinical service. Our guide to clinicians and telehealth platforms explains why that distinction matters when identifying whom to contact about a treatment decision.

The general terms limit service to adults at least 18, or the local age of majority if higher, in states where service is offered. They also require patients consulting a provider to be in the same state as the shipping address in their account. The consulted documentation does not establish a complete state-by-state program roster or current clinician availability. A person traveling or using a different delivery address should resolve that location requirement before scheduling, rather than assuming an online appointment is location-independent.

The first consultation is an evaluation, not a prescription purchase

Enrollment begins with a brief questionnaire about goals and program personalization, followed by choosing a provider. Sesame then describes a video consultation covering medical history and weight-loss goals. The clinician develops a treatment plan and can order laboratory testing if metabolic information is needed. Preparing a medication list, relevant history and questions is more useful than treating the questionnaire as final approval; our first-consultation preparation guide offers a framework for that conversation. The documents do not promise that every consultation produces a prescription.

Sesame describes ideal medication candidates as people with a BMI of at least 30, or at least 27 with weight-related health conditions. Those are program descriptions, not an individual eligibility determination. Its page also lists pancreatitis history, personal or family history of medullary thyroid cancer, MEN2 and type 1 diabetes as GLP-1 restrictions. These are provider-stated screening boundaries, not a complete clinical assessment. The controlling service terms say medication is prescribed only when medically necessary and at the provider’s discretion.

Understand what the care subscription buys

Success by Sesame starts at $59 per month with an annual subscription, including telehealth visits, unlimited messaging and ongoing care from a chosen provider. The advertised month-to-month price is $99, and clinicians set their own prices, so some charge more. Medication is excluded. The page also offers three- and six-month plans, billed in advance every three or six months, while annual billing occurs once every 12 months. The monthly equivalent is therefore not a promise that the annual option is collected one month at a time.

The short-term billing description needs particular clarification: one passage says the $99 plan is billed every month, while another specifies advance billing every 28 days. Four weeks is not a calendar month. Laboratory benefits also have boundaries. Included testing must occur at Quest Diagnostics, excluding facilities in Arizona, Hawaii, North Dakota, New Jersey, New York, Oklahoma, Rhode Island, South Dakota and Wyoming. In those states, laboratory facility fees are additional. Before the clinician orders testing, confirm the location and who will collect any separate charge.

Medication choice changes both the prescription and the bill

Sesame’s product pages distinguish routes and presentations rather than offering one interchangeable GLP-1 package. Its Wegovy page describes injectable and oral semaglutide. It advertises oral tablets without insurance at $149 for a month’s supply of the 1.5 mg or 4 mg presentations, and $299 for the 9 mg or 25 mg presentations, separately from membership. These are price categories, not dosing instructions. The page directs patients seeking reduced cash pricing for brand-name Wegovy to have their provider send the prescription to NovoCare pharmacy.

The injectable Wegovy card advertises $199 per month for the first two months for new patients, then $349 per month. The accompanying promotion covers new self-pay patients in the United States whose prescriber authorizes Wegovy or Ozempic injections, effective November 17, 2025, through December 31, 2026. Its $199 price applies to the first two consecutive treatment months at the 0.25 mg or 0.5 mg presentations. People enrolled in federal or state healthcare programs with prescription coverage are excluded. The discount covers medication, not subscription or service fees.

The pharmacy destination is part of the treatment logistics

The Zepbound page lists single-dose injection pens, multidose KwikPens containing a month’s supply per pen, and single-dose vials requiring a separate needle and syringe. Its LillyDirect cash prices for vials are $299 monthly for 2.5 mg, $399 for 5 mg, and $449 for 7.5, 10, 12.5 or 15 mg. For KwikPens through LillyDirect, it lists $299, $399 and $499 for the 2.5, 5 and 7.5 mg presentations, respectively, and $699 for 10, 12.5 or 15 mg. These are medication charges outside membership.

The general program page instead lists $398 for the 5 mg KwikPen and $698 for the higher presentations, making pharmacy confirmation preferable to assuming either page controls checkout. More broadly, Sesame advertises same-day local pickup if medication is prescribed, or home delivery, and its terms name Truepill as a delivery partner. That does not establish which pharmacy handles every product or guarantee stock or arrival. The pharmacy-handoff checklist helps separate prescription routing from dispensing, payment and delivery arrangements.

Insurance assistance does not settle eligibility or coverage

Sesame says it does not bill health insurance for the weight-management program, and that patients may be able to use insurance for prescribed drugs. It also says the provider will work directly with the insurer on prior-authorization paperwork. Those are distinct parts of the pathway: paying privately for visits does not determine whether a drug is covered, and completing paperwork does not guarantee authorization. A patient needs confirmation of the medication benefit separately from confirmation that the clinician is available for a cash-pay consultation.

There is a more consequential tension in the documents. Broad marketing language about access regardless of insurance sits alongside telemedicine terms requiring users to certify that they are not federal healthcare-program beneficiaries, with Medicare, Medicaid and TRICARE given as examples. Anyone affected should obtain a clear eligibility answer before paying. Separately, Sesame says most subscription services may qualify for HSA or FSA reimbursement and offers an itemized bill through support@sesamecare.com. That is a documentation service and a conditional reimbursement claim, not a guarantee about an account administrator’s decision.

Follow-up is documented, but its operating details are thinner

The continuing-care offer combines video care, provider messaging, weight tracking and tools addressing sleep, exercise and eating habits. The Wegovy page specifically promotes requesting refills by messaging the provider rather than booking a dedicated refill visit. That creates a documented contact route, but not an automatic refill entitlement. The consulted pages do not establish messaging response deadlines, support hours or a mandatory follow-up schedule. Before relying on the program for ongoing prescriptions, ask how far ahead to request review and who handles messages when the selected clinician is unavailable.

Clinical follow-up should concern response and tolerability, not simply the next payment; NIDDK describes medication selection as a discussion of benefits, side effects, health conditions, other medicines and costs. Delivery problems require a separate plan because Sesame does not document carriers, shipping charges, cold-chain procedures or replacement rules here. FDA explicitly advises not using an injectable GLP-1 product that arrives warm or insufficiently refrigerated, and contacting the provider or dispensing pharmacy. Our guide to ongoing support helps frame the division between routine communication and unresolved care needs.

Plan the exit before the next charge

The program’s refund policy offers a full refund when enrollment is canceled at least three hours before the initial visit starts. Once that visit has occurred, it says the first month is not refundable. For subsequent charges, the program page describes self-cancellation at any point, provided it occurs before the next billing cycle, and says previous months are not refundable. This distinguishes avoiding a future renewal from recovering money already paid. It does not clearly explain all consequences for unused portions of prepaid three-, six- or twelve-month plans.

General membership terms use a different procedure: email support@sesamecare.com or call +1 (877) 947-6411 at least 48 hours before the applicable renewal processing date. They say access continues through the paid term after cancellation, without a prorated refund. Because the documents do not fully reconcile these instructions, obtain the applicable renewal date and cancellation procedure in writing rather than relying on the less restrictive wording. Ending a subscription also leaves a practical care question: who will manage future prescription requests once paid access expires?

Questions & answers

Does paying for Success by Sesame guarantee a GLP-1 prescription?

No. The subscription provides access to documented care services, including evaluation and ongoing communication. Sesame’s terms leave prescribing to the clinician’s discretion and require medical necessity. The intake questionnaire and advertised candidate criteria do not substitute for that decision. Ask what happens financially if the clinician recommends a different approach, because the first-month refund policy changes once the initial visit has occurred.

Will Sesame send my medication, or can I use a local pharmacy?

The program advertises local pharmacy pickup or home delivery when medication is prescribed. Sesame itself is not a pharmacy; its terms name Truepill as a partner for facilitating delivery. Specific cash-price pathways also point to NovoCare for Wegovy and LillyDirect for Zepbound. Confirm the destination before the prescription is sent, since a preferred local pharmacy should not be assumed to offer a manufacturer-channel price.

Sources: [7] [4] [6] [5]
Are laboratory tests included everywhere Sesame is available?

No. The stated benefit applies to testing at Quest Diagnostics and excludes Quest facilities in AZ, HI, ND, NJ, NY, OK, RI, SD and WY. In those states, laboratory facility fees are additional to membership. The clinician may order testing when metabolic information is needed, but that does not make every laboratory location or test purchase included. Confirm the order, facility and charge before attending.

Sources: [7]
Can I request a Wegovy refill without another appointment?

Sesame’s Wegovy page says patients can message their provider for refills instead of booking a specific refill visit. That describes the request process, not guaranteed approval or immediate turnaround. The public documentation does not specify message-response deadlines or a required follow-up frequency. Ask the selected clinician when a new assessment would be necessary and how much lead time to allow before a prescription runs out.

Does Sesame offer options other than injectable GLP-1 medicines?

Its pages describe oral Wegovy and market Foundayo, identified as orforglipron, as an oral GLP-1 option. The program also lists diabetes medications that may cause weight loss, including Ozempic, Mounjaro, Rybelsus, Victoza and metformin; that is not a statement that every listed medicine is a GLP-1. Sesame says its providers do not prescribe controlled weight-loss medications. Product descriptions and regulatory claims on these pages were not independently verified here.

How should I cancel without assuming I will receive a refund?

Confirm your renewal processing date and the procedure applicable to your plan. The general terms require requesting cancellation by email or phone at least 48 hours beforehand, despite the program page’s broader self-cancellation wording. Keep confirmation of the request. The terms preserve access until the paid term ends but exclude prorated refunds; the separate initial-visit policy offers a full refund only when cancellation occurs at least three hours before that visit.

Our assessment

Success by Sesame documents a recognizable continuing-care pathway rather than merely selling access to a prescription request: clinician choice, video evaluation, conditional laboratory testing, messaging, tracking and insurance paperwork assistance all have defined places. Separating medication purchases from membership can also make responsibilities easier to identify, provided patients confirm the pharmacy channel and exact product price. The weaker points are operational and contractual clarity. Public documentation leaves response times and delivery contingencies unresolved while giving inconsistent accounts of monthly billing, cancellation and some medication prices. Federal-beneficiary restrictions also require attention despite broad access language. The most useful enrollment preparation is therefore a written confirmation of the selected clinician’s fee, billing interval, laboratory coverage, pharmacy destination and cancellation deadline. Those answers matter more to continuity of care than the lowest advertised monthly figure.
